Yizkor Memorial Book

Ohav Sholom is putting together the Yizkor Memorial Book for New Year 5786.

If you would like to include the names of your loved ones, please fill out the form below.  The cost is $18 per name for each of the first two names and $10 for each name thereafter. 

Please note that all submissions must be received by September 11, 2025.  This is a FIRM deadline, as we need time to proofread before printing.
