Yizkor Memorial Book

Ohav Sholom is putting together the Yizkor Memorial Book for New Year 5784.

If you would like to include the names of your loved ones, please fill out the form below.  The cost is $18 per name for each of the first two names and $10 for each name thereafter. 

Please note that all submissions must be received by August 30, 2023.  This is a FIRM deadline, as we need time to proofread before printing.

IMPORTANT:  If you signed up for auto-renewal last year, you do not need to re-submit your names this year.  If you have new names to submit this year, please follow the instruction on the form to ensure you are charged correctly.
